I have spoken with Ziggo today about being misled by their sales team at the time of opening a new internet and TV packaged account - I was taking advantage of their 12 months half price offer at the time only to find out the half price came to an end in month 6.
After discussing the above with Ziggo's administration department this afternoon, I was informed that the only way to pursue this would be to put in a formal complaint via their website through submitting a form. After endlessly searching the internet, I have not been able to find such form.
Does anyone know how I can access this form, or how to submit a formal complaint? The administration department said they do not have access to certain systems that provide further information such as call logs or customer communication and that it would only be the complaints department that can access these systems.
I have not had a great experience with Ziggo so far.

There is no such form (anymore).
Thank you for (finally) clarifying that "there is no such form anymore". So I was misinformed by Ziggo's Customer Service department today - great!
